DECLARATION OF EXEMPTION FROM IRB REVIEW

The DUHS IRB has determined that the following protocol meets the criteria for 
a declaration of research not involving a human subject (45CFR46.102(f).

This Declaration of Research Not Involving a Human Subject is in effect from 
7/30/2007 and will remain in effect until 1/1/2100 ("Termination Date").  
While this date differs from my earlier comments to you, regard this as the 
correct termination date.  

Please be advised that any change to the proposed research will require a 
re-application to the IRB.  If this protocol will be active by the Termination 
Date, please submit an application for renewal of the Declaration of Research 
Not Involving a Human Subject to the DUHS IRB at least thirty (30) days prior 
to the Termination Date.
